Purpose of collecting data

You (the user) hereby agree that this data will be stored and processed for the purpose of establishing contact. Should discussions continue then you understand that further data and correspondence will also be held by the Data Controller who is responsible for data records and processing and reporting any data breaches.

We may collect and process the following data about you:
Information that you provide by filling in forms on our website. This includes information requesting further services, and any subsequent data that you provide in further communication or attendance;
If you contacted us, we may keep a record of that correspondence;
We may ask you for information for research purposes, although you do not have to respond to them. Any survey data will be made anonymous unless permission has been granted to make it identifiable personal data;
Information that we require in order to process invoices, receipts and to file tax returns.

We will ensure that the data held is accurate, adequate, relevant and not excessive.

Personal client data that may be relevant for an agreed Programme with Selena Clay or Dyslexia South Wales will be stored and utilised and may have to be shared with authorised bodies such as Davis® licensing authorities/personnel. For any other purposes, identifiable data will be anonymised.

The data that we collect from you will be stored securely so as to prevent anyone without appropriate authority accessing it. All data will be password protected or locked away.

Information will only be kept in a personal identifiable state for as long as it is required to provide support and for six years after final contact, unless there is agreement to keep for longer. Anonymised data may be retained for longer periods for research purposes.

All other data will be destroyed or anonymised unless required for tax records. Tax records will include copies of invoices and receipts.

We use information held about you in the following ways:

To provide you with information, products or services that you have requested;
To provide you with information, products or services that we feel may be of interest to you or be helpful to your development or support;
To carry out our obligations arising from any contracts entered into between you and us or administration purposes including invoices and receipts;
To enable us to compile statistics, or to assist other organisations to do so, provided that no statistical information that would identify you as an individual will be published, unless you have given specific permission to do so.

You should be aware that you can request to withdraw your consent for us to hold your data in a personally identifiable state at any time by emailing selena@dyslexia-southwales.co.uk

 

Professional services described as Davis®, including Davis® Dyslexia Correction, Davis® Symbol Mastery, Davis® Orientation Counseling, Davis® Attention Mastery, Davis® Math Mastery, and Davis® Reading Program for Young Learners may only be provided by persons who are trained and licensed as Davis Facilitators or Specialists by Davis Dyslexia Association International.
